- Issue of fresh passport for the first time
- Fully completed and signed application form CS01
- 4 photographs (see type of photos )
- Reason of request statement (See form CS06)
- Birth certificate or education certificate or other similar certificates originated from Ethiopian government and attested by Ministry of foreign affairs of Ethiopia (copy and original)
- Or data of three Ethiopians living in Ethiopia who can testify about your nationality
- For those who stayed in Ireland five or more years affidavit declaring that the applicant has not sought or obtained any other nationality (see form CS16)
- To have fingerprint captured in front of embassy staff
- Appropriate fee (see service fees)
- Passport is valid for five years from the date of issue
- Issue of fresh substitute for expired/old passport
- Fully completed and signed application form CS01
- 4 photograph (see type of photos )
- Old passport (copy and original)
- For those who stayed in Ireland five or more years affidavit declaring that the applicant has not sought or obtained any other nationality (see form CS16 )
- To have fingerprint captured in front of embassy staff

- Passport is valid for five years from the date of issue
- Issue of fresh substitute for lost/damaged passport
- Fully completed and signed application form CS01

- Affidavit giving detailed explanation of the circumstances under which the passport was damaged/lost (see form CS05)
- Report lodged with the nearest Police Station for lost passport
- Copy of old passport
- Or birth certificate or education certificate or other similar certificates originated from Ethiopian government and attested by Ministry of foreign affairs of Ethiopia (copy and original)
- Or data of three Ethiopians living in Ethiopia who can testify about your nationality
- For those who stayed in Ireland five or more years affidavit declaring that the applicant has not sought or obtained any other nationality (see form CS16)

- Fully completed and signed application form CS01
- 4 photographs (see type of photos )
- Current valid passport of Ethiopian parent/s (copy and original)
- Birth certificate or education certificate or other similar certificates originated from Ethiopian government and attested by Ministry of foreign affairs of Ethiopia (copy and original)
- Or birth certificate from the country of birth (copy and original)
- Affidavit given by the parent/s declaring that the child has not sought or obtained any other nationality (see form CS13)
- To have fingerprint captured in front of embassy staff for those 14 years and above
- Up to the age of 14 years (exclusive) don’t require finger print
- Appropriate fee (see service fees)
- Passport is valid for five years from the date of issue
- Change of name on the passport
- Same as new Passport cases, as it requires issue of a new Passport
- Court order about the change of name in Ethiopia
- In case of change of maiden name on marriage, marriage certificate (copy and original) and court order about the change is required
- To enter children’s data on passport
- This service is not available any more
- Children may have their own passport
- Renewal of expired passport
- This service is not available any more
- If passport expires it should be replaced (see Issuance new passport to replace expired/old passport )
- Amendment on passport
- This service is not available any more
- If amendment is required the passport should be replaced (see Issue of fresh substitute for expired/old passport )
